QOL Pharma, a social enterprise that began in March 2016, functioning as the first Comprehensive Center for Care of Patients with Chronic Conditions in the country. Our goal is to become the main family affected by these diseases, to facilitate the acceptance of the condition and accompany them in the resizing of the daily dynamics that allow us to generate a new concept of quality of life for both the patient, S) caregiver (s) and family nucleus, with multidisciplinary studies and discussion tables of each individual case. We start from a comprehensive counseling model that contemplates the review of the cases considering an assessment of the general health of the patient and their pharmacological treatments; So that on the results, a multidisciplinary team formulate a proposal of hierarchical health plan according to the priority required to begin to enjoy a better quality of life on the part of the patient. An agenda of educational workshops for the different actors of the process and its family nucleus, with a view to empowering them on the condition, provides benefits such as access to medical supplies and specialized drugs at preferential prices.
